I blame the father, the “child” who was acting as a safety officer/instructor (IIRC, he was just 14) and the promoter that allowed a child to act as a safety officer/instructor.

I blame them all. But what about the “individual responsibility” that yuo guys are always touting? Does it have a lower age limit?
